About us
Together 21 is a project run by health and wellbeing charity, Pioneering Care Partnership. The service has been supporting children and young people with Down syndrome and their families since 2017.
The Pioneering Care Partnership (PCP) is an award-winning Health and Wellbeing charity based in Newton Aycliffe. PCP’s Mission is: ‘Health, Wellbeing and Learning for All’. The organisation’s purpose is ‘To improve health, wellbeing and quality of life for people disadvantaged by disability, age, ill health and/or caring responsibilities, and reduce inequalities in health'.


At the heart of Together 21 is the principle of Learning, Fun and Friendship. When families join Together 21 the children and young people with Down syndrome along with their families, acquire essential skills that will equip them to meet the future with confidence. Health and education professionals are warmly welcomed to see and share ideas about the project. These professionals are enabled to take the ideas into their own settings.
The activities on offer at Together 21 provide access to a carousel of support for each child, tailored to their individual learning needs including:

Along with our strong focus on learning and development, Together 21 strives to raise the profile, expectations and possibilities of our children with Down syndrome within their family settings, school communities and local communities.
We believe that all children have the ability to learn and play an active and meaningful role in their families’ lives and communities.
